Advanced Physical Medicine is looking to hire a knowledgeable individual who is dependable, responsible, friendly and most importantly dedicated in well begin of our patients.. We are a multi-disciplinary practice, working with the MD, PT, DC, DPM and other specialist to help care for patients involved in work related injuries, auto injuries, personal and sport injuries(ect.) We work together as a team to diagnose and treat each individual as an individual, paying close attention to the needs and focusing always on the goal.

You will work with a MD & DPT to help with patients involved in an auto accident, work injury, slip and fall ect.

We are looking for an individual who is self motivated and responsible. We want an individual who is looking for long term commitment and growth with the company.

If you are Licensed in IL we will help and reimburse for IN licensing cost.

Requirements

Review daily schedule prior to start of the day
-Check how many patients you have for the day. Review how many first-time patients are beginning
exercises. 
New Patients
-Check to see if you have the provider's order. Review diagnosis and plan of care.
New Patient Flow-Sheet
-Add New Patients to your flow sheet binder Or use ChiroTouch to keep tract of exercises and time
Modalities
-Under the direction and supervision of physical therapists, physical therapist assistants treat patients through exercise, massage, gait and balance training, and other therapeutic interventions.
Documentation
- Record the patients' progress and report the results of each treatment. (SOAP Note) uploaded to the patient's EPF within 48 hours.
Communication with Provider & Case Manager/Follow-Up
- Review the patient's treatment progress and visits. Review the Plan of Care.
